All passengers travelling on JetAir flights operated by Air Nostrum to or from Madrid are required by the Spanish authorities to complete an online health declaration form.

Allow a few moments for the web form to display.  The form may be completed in English or Spanish.

The form should be completed no sooner than 48 hours before the flight departure.
